Are you a young person aged 14 -25 who may need support from health services or social care in the future?
Dorset Council know that transition to adult life needs to be better supported by good information, advice and better services.
To get this right they are committed to listening to parents, carers and to young people. In June, they launched a transitions programme where they captured feedback on the things that are important to parents and carers now, and in the future.
